When you get to this website, click on "YES" to the security question, scroll down to blue "FORM 2000C" in the middle of the page, enter the information required.  Scroll down and click "SUBMIT at the bottom of the page to make sure the FCC receives the complaint information. Retain the Acknowledgment Number they send back as this will be your complaint number that can be used to track your complaint later.
